DfT statistics reveal that the biggest single contributory factor on the
part of the bicycle in accidents involving bicycles is the cyclist failing
to look properly (25%, in accidents involving a bike), followed by failing
to judge the other person's path or speed (10%), the cyclist entering the
road from the pavement (8%), and careless or reckless behaviour (8%).

Total 51 per cent, so if cyclists improved their own use of the roads the
KSI rate would be halved at a stroke.

Look at the source of your misquoted figures, Table RAS50005.

It is not about car/cycle accidents. It is called "Table RAS50005:
Contributory factors reported for vehicles in reported accidents by
vehicle type: GB 2010"

Yes the thread title needs changing to be more accurate.
