Failure to make proper observation of the road ahead - you don't just
look at where you want to be going, you have to look at where other
road users may be coming from too, especially at junctions. Although
some of those wide-open spaces on foreign roads are particularly
challenging - three or four lanes on each side in each direction and
nothing much to show turning vehicles where to go. "Shared space"
doesn't really work on this scale.
