Default Derailleur adjustments

Is there a good guide to adjusting derailleur gears on the net anywhere?

On a child's bike with six gears and a gripshift (Shimano Revoshift):

1st looks as though it's trying to jump the chain inwards, off the
sprockets
2nd and 3rd run smoothly and quietly
4th wants to jump into 5th
5th tries to jump into 6th
6th seems happy enough

It seems to me that the shift mechanism is moving the derailleur too far
between gears, and that this needs to be adjusted slightly.

Any suggestions?

The amount of shift on index gears is not adjustable, maybe you have the
wrong shifters or maybe the derailleur or its hanger are bent.
