Default Bottom bracket spindle (where to find one these days?)

I'm looking for a bottom bracket spindle that fits an Italian width,
but is narrow (to fit the chainline on a fixed gear), however, bottom
bracket spindles seem a rare beast these days, especially for Italian
shell widths. If anyone has an idea, I'd appreciate it.

MANY genuine 'bike shops' still can get and have many Italian BBs in all types
of spindle lengths and configurations.

TA, Campagnolo, shimano, FSA, Sugino to name just a few. MO does not decide
what is available.
